Identity Theft Protection
Posted on Jul 1, 2011 In order to protect our patients and as a response to new Federal guidelines, we have implemented an Identity Theft Program. Its purpose is to protect all patients from medical identity theft as well as making sure the medical care provided by our staff is being rendered to the appropriate individual.
We are asking that all patients provide photo identification at the time of your visit as well as allowing us to take a photo for your medical record. Having a photo in your record will eliminate the need of presenting photo ID at subsequent visits.
